Apply for Services

 

ADD-NI accepts referrals from a range of professionals and also encourages families to make referrals for services. To make a request for ADD-NI’s services a referral form must be completed on this website or downloaded and sent by post to the support centre 33 Ballynahinch Road, Carryduff, BT8 8EH. 

 

There are three referral methods 
 

  • Parental Request for services - Parents can fill out the "Parental Request for Services" form to request ADD-NI services on behalf of their child or young person.  This applies to young people under 18 years of age.
     
  • Self Request for Services - An adult can fill out the "Self Request for Services" form to refer themselves for ADD-NI’s services for example for ADD-NI’s Parenting Programme.  This applies to those who are over 18 years.
     
  • Professional Request for Services - A professional for example Paediatrician, Social worker or representative from the organisation acting on the behalf of a child, family or individual can make a referral for ADD-NI’s services.  On this form they must indicate what services they are requesting for example if you are referring a young person, adult or specific family member.

 

 

All who come in contact with ADD-NI are to be valued as an individual in their own right with talents, skills and abilities, as being unique and worthy of respect.

It is our experience that in order to afford long term combined improvements for each individual and their family, we deal in both a combined person centred approach and/ systemic manner.
